2. Appointment requests are requests, not confirmations

Submitting an appointment request through the Website does not schedule an appointment. A member of our team will contact you to confirm availability, verify insurance, or propose an alternate time. Appointments are confirmed only after our staff communicates a confirmed date and time.

You must be at least 18 years old, or the parent/legal guardian of a minor, to submit forms on the Website. You agree that information you submit is accurate to the best of your knowledge and that you have authority to submit it. Do not submit information about another adult without their consent.

When you provide contact information you may receive appointment-related communications from us by phone, email, and text message. Text-message practices — including message types, frequency, and how to opt out — are described in our SMS Opt-Out Policy.

5. Insurance and financial responsibility

Insurance verifications performed as a courtesy are estimates based on information provided by your insurer and are not a guarantee of coverage or payment. Financial responsibility ultimately rests with the patient or responsible party in accordance with the practice’s financial policy, which is provided at intake.

The Website may link to third-party services, including our patient portal hosted by athenahealth. We do not control third-party services and are not responsible for their content, policies, or availability. Your use of those services is governed by their own terms and privacy notices.
